Orthopoxvirus infection is a DNA virus with endemic characteristics in West and Central Africa, where it has been responsible for sporadic cases in humans since 1970. The first cases of Monkeypox infection were reported in Europe in May 2022. In Peru, on July 1, 2022, there were already 3 confirmed cases in Lima and the epidemiological alert of the risk of spread to other regions was published. A clinical case with atypical symptoms is presented, with risk factors; having a positive result for monkeypox confirmed the day after the sample was taken. Due to the high probability of transmission in risk groups described, sexual contact being the main route of transmission, a rapid diagnosis and isolation of the patient is recommended, as well as continuous monitoring that includes contacts.
