Intrathoracic ribs are atypical congenital anomalies that occur due to fusion failure during the embryogenesis period, they are usually incidental findings and do not present symptoms. We present the case of a 67-year-old female patient with intrathoracic rib dependent on the proximal portion of the left fourth costal arch diagnosed by chest tomography with 3D reconstruction. This patient would be the first case reported in the peruvian literature.
