Endocardial pacemaker implantation in a 7-month-old infant and long-term outcome: A case report

Abstract

Objective: We report the case of a seven-month-old weighing 5.9 kilograms, complete atrioventricular block 3 months after surgical closure of a ventricular septal defect (VSD), in whom a single-chamber endocardial
pacemaker was implanted. Clinical case: Seven-month-old infant patient, postoperative for VSD closure. A 12-lead electrocardiogram showed complete AV block with a supra-Hisian rhythm rate of 38 beats per minute (bpm), and
a 24-hour Holter monitor recorded average rates of 42 bpm. Because data of low cardiac output with pulmonary congestion were observed in the following days, without reversal of complete AV block, it was decided to implant a single-chamber pacemaker endocardially. The patient evolved without complications. Conclusion: Endocardial
pacemaker implantation, even in children weighing less than 15 kg, is a reasonably effective and safe option in experienced centers, and can also improve the prognosis of these patients
