We present the case of a 76-year-old patient with a history of arterial hypertension and smoking, who came for evaluation due to dyspnea and chest pain of three weeks' duration. The electrocardiogram showed signs of necrosis in the lower face. With multimodal imaging, the diagnosis of giant ventricular pseudoaneurysm was made and surgical correction was decided, unfortunately the patient died of pneumonia due to SARS-CoV-2.
