Perianal fistulization is an inflammatory condition with a multifactorial cause, which produce significant morbidity in patients, having a high rate of recurrence after treatment. Magnetic resonance imaging (MRI) has become the technique of choice in the evaluation and follow-up of these patients, over surgical exploration and other imaging techniques. It provides precise anatomical information the anal sphincter complex, as well as the trajectory of the fistulas and their anatomical relationship with the different structures of the pelvis. Allows adequate therapeutic planning reducing the recurrence or persistence of the inflammatory process, as well as avoid possible complications derived from surgical treatment. MRI has also shown high utility in evaluating the response in those patients receiving antibiotic or immunosuppressants treatment.
